SUMMER/FALL 2026

Welcome to the start of the 2026 athletic year!

Many of our sports offer SUMMER activities starting June 1st. To participate athletes must be fully cleared through Final Forms.

2026 Registration Opens May 15, 2026
All athletic registration is done through FinalForms.
Athletes must be fully registered and cleared to participate.
  • Returning users: Log in to update your information and register for 2026 school year and add sports.
  • New users: Visit https://universityplace-wa.finalforms.com, create your account, add your student(s), and complete all required forms.
Medical Clearance
School and District nurses clear all student-athletes medically. This does happen manually and will take some time. These will all be cleared by the first day of practice. If your child has a need to carry life saving medicine, please submit doctors orders as soon as possible. Failure to do so could result in not being able to participate.

ASB and Fines
All student-athletes must purchase an ASB card to participate in athletics. Please DO NOT purchase the 2026-2027 ASB until after August 1, 2026. All fines must be cleared prior to purchasing an ASB.

Transfer Students
Only current UPSD students may participate in our athletic programs. If your student is new to the district or you have questions, please email me at svick@upsd83.org.

Key Dates to Remember
  • Athletic Registration Opens May 15, 2026
  • Football Begins: August 19, 2026
  • All Other Fall Sports Begin: August 24, 2026

2026-2027 Sports Registration Opens May 15, 2026

Final Forms is our online forms and data management service. Final Forms allows you to complete and
sign athletic participation forms for your students. The most exciting news is that Final Forms saves data
from season-to-season and year-to-year, meaning that you will never need to enter the same information
twice! FinalForms also pre-populates information wherever possible, for each of your students, saving
you time. You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

ALL parents and athletes will be required to use Final Forms.  
In addition, all athletes will need to have a current physical, clear all fines and purchase an ASB.
